The visual story combines three lenses: direct damage to document integrity, how often the problem appears, and how much cleanup it creates after conversion. Bubble size represents frequency, while the composite priority index multiplies damage, recurrence, and fix effort to surface the costliest remediation risks.

Ordinal tiers are used for the operational dimensions: Frequency maps from Low to Very High as 1–4, and downstream fix effort maps from Low to High as 1–3. That keeps the charts comparable without flattening the categorical labels visible in hover.
